Outpatient visits require itemized invoices with diagnostic information, while hospital stays require the final invoice issued after treatment. Medications require both a prescription and a pharmacy receipt, and aids or physical therapy require a doctor's referral. Claims made within the first 31 days of your policy require additional documents to verify waiting period waiver eligibility.

What documents are required for an outpatient appointment claim?

You must submit an invoice that includes the diagnosis and an itemized breakdown of costs for each service provided during the visit. A valid invoice lists each service on a separate line with a service code, a description of the treatment, and the corresponding cost in euros. For example, a typical private medical invoice includes columns for:

  • Date (Datum)

  • Service code (Ziffer)

  • Service description and reason (Leistungstext und Begründung)

  • Rate factor (Faktor)

  • Amount charged in euros (Betrag €)

What documents are required to submit a claim for a hospital visit or inpatient stay?

You must submit the final invoice issued by the hospital after your treatment is complete, which reflects the full breakdown of all treatments and costs.

The hospital may ask for a prepayment on the day of your visit and may provide a receipt, but this receipt is only a cost estimate — the final amount may differ, and in some cases you may be owed a refund.

For inpatient stays, the final invoice must be accompanied by the discharge notification and discharge report.

The following are not accepted as substitutes for the final invoice:

  • Prepayment receipts collected on the day of your visit

  • Partial payments that do not reflect the total final cost

What documents are required for a medication claim?

You must submit both of the following:

  • The prescription issued by your doctor

  • The payment receipt from the pharmacy

A standalone receipt without a prescription, or a prescription without a receipt, cannot be accepted.

What documents are required for aids or physical therapy claims?

You must submit the following:

  • A referral from a medical doctor

  • The relevant invoice for the aids or physical therapy received

A referral is not required for general specialist visits — only for claims related to aids or physical therapy.

What additional documents are needed for claims made within the first 31 days of my policy?

If you submit a claim within the first 31 days of your policy, you must attach supporting documents so we can verify whether you qualify for a waiting period waiver:

  • If your waiver is based on arriving in Germany within 31 days of your policy start date, attach a transportation receipt (such as a flight or train ticket) showing your name and arrival date.

  • If your waiver is based on continuous prior coverage, attach proof of your previous insurance showing your name and the dates of coverage, with no gap before your new policy started.

Which documents are not accepted for expat health insurance claims?

The following documents cannot be used to process any expat health insurance claim:

  • Invoices that lack diagnostic information or an itemized cost breakdown

  • Prepayment receipts or partial payments

  • Cost estimates

  • Payment reminders

  • Standalone payment receipts without an accompanying invoice or prescription

  • Invoices from non-medical practitioners, such as acupuncturists
