The invoice must include:

  • Your details

  • Diagnosis/reason for the visit

  • A list of the performed procedures and corresponding costs

The invoice will not be accepted if:

  • It only mentions "consultation" with no specification of what the consultation was for. Please ask your doctor to be as specific as possible.

  • It only contains a total amount/flat fee. If the total cost of the treatment is €100 euros, the invoice must include the breakdown of the €100.

If the invoice is not already in German, English, or Spanish:

Before submitting a new claim, you'll need to translate the documents to English.

All pages of the invoice (as well as referrals and any supporting documents) must be translated. Please provide both the translated and untranslated versions when submitting the claim.

It doesn't have to be an official translation, an online translation is sufficient.

  • We recommend using any free online pdf translator.

  • If that's not possible, you can also translate the documents yourself in a separate pdf document.
